Updating Contact Information

Please keep the office apprised of any changes to your primary and emergency contact information so that we can reach you quickly if needed.

Primary Contacts

To update the following, please email the new information to our Office Manager, Alicia Baker and our Office Assistant, Takeera Suber.

• Address changes
  Please note, we will need proof of residency in the form of a current lease, deed, or utility bill. (Bills from cell phone or internet providers cannot be accepted.)

• Parent/Guardian Phone(s) -- home, mobile, work
